For a woman, taking hormones eventually makes everything down there start to function differently and more like the tissues in female organs, and post-op, everything is still intact, but just in a different configuration. So erections kind of have to happen a different way I don't know everything about that, since I'm a trans girl and not a trans guybut they certainly can achieve orgasms for the same reasons as a trans woman.
